This form is to be completed by the property owner. Form is to request that the District send the bills to a property manager or property management company. The owner will receive copies of any past due statements. If a lien is filed due to non-payment of sewer charges, the property management information will be permanently be removed from the account.

Midway Sewer District is fully committed to providing accessible facilities, elements and
channels of communication to all members of the public. As part of this commitment, Midway Sewer District
